10/25/2023 EnchantaVaughn Approved Behavior: QMHPA arrived at the home to conduct individual session with the Chevelle. QMHPA inquired about her morning. Chevelle discussed with QMHPA her morning was going well. QMHPA informed Chevelle the topic of today’s session which was to continue to assist her with being able to identify coping skills and understanding her mental health triggers. Intervention: QMHPA begin session by inquiring about Chevelle's emotional well-being. QMHPA through role play exercise and sharing personal experiences helped QMHPA process how her decision making and choices can impact her life and others. QMHPA explained to QMHPA that this skill will assist her in identifying what would be an appropriate course of action to take in determining how to pattern her life and the lives of others she comes into contact with. QMHPA encouraged QMHPA to use those interpersonal skills such as team work, problem solving and consistency to help decrease her anger and anger outbursts. QMHPA used positive self-talk to encourage using relaxation techniques as often as it becomes necessary through modeling and role-playing. Response: Chevelle was open to suggestions and working on her anger for today's session. While exploring Chevelle’s current mood, she reported to QMHPA, she was feeling good. Upon arrival Chevelle was in the kitchen cooking. QMHPA shared that she was in a good mood as evident by her smiling and engaging in conversation with QMHPA. Chevelle admitted that she has to make decisions for herself and her family as well as other in her family even though they get on her nerves and they are not there for her the way that she is there for them. Chevelle was encouraged to make sure that she communicates her needs with others using non aggressive methods. Chevelle acknowledged that she has to be mindful of her actions regarding her choices and decisions. Chevelle shared that her actions with respect to the choices and decisions she makes can be thoughtless and at times done out of spite. Chevelle was able to process through communications how she can use her interpersonal skills in order to connect with her health care providers and her family more effectively so everyone can have their needs met. Chevelle admits that she needs to work on her people skills so that she can decrease some angry outburst and some depressive moments. Chevelle was commended for being proactive regarding her mental health needs. Plan: QMHPA will continue to engage Chevelle in more role-playing activities assist her with learning how to respond composedly in stressful situations that may occur (1-2 times weekly).
